Before you begin
Please note reports will be actioned 8.30am to 5.00pm business days. If you are submitting a report about an environmental emergency, please contact the department on 1300 130 372 and select Option 2.
To complete this form, you need the location details of the incident and any supporting evidence. Please provide as much detail as possible to help our investigation.
Make sure you have prepared any supporting evidence (e.g. an odour log sheet , noise log sheet, other documents, photographs, audio, or video). Log sheets are generally used for problems that are not ‘one-off’ events, and detailed reports help us focus our inspections. The more information you provide, the greater our ability to track patterns and trends.
